Both Advantra Z and Xenadrine EFX contain extracts of Citrus aurantium, also known as bitter orange extract. The researchers found that like ephedra-based products, the two ephedra-free supplements increased the heart rates of all research subjects. One of the 10 research subjects also experienced increased blood pressure. Ephedra, an herbal stimulant, was banned in the United States after various reports linked the substance to strokes, heart attacks and at least 155 deaths.

Citrus aurantium contains the compound synephrine. Like ephedra, synephrine stimulates a person's central nervous system and may contribute to increased metabolism. Researchers question both the safety and effectiveness of supplements containing bitter orange extract and suggest consumers be wary of any herbal supplements until further research has been completed.
