The Board also criticized the pilot, Wayne Price, reporting that he had been warned by two passengers that the wings of the Cessna Caravan 208 had ice on them, but he made the decision to fly the plane anyway.

The plane crash occurred on Jan. 17, 2004, killing eight hunters from Ontario, Price, and his girlfriend.

The total weight of the plane was 4,454 kilograms when it crashed, the report said. But the Cessna's aircraft manual recommends that it weigh no more than 3,878 kilograms when flying in icy weather.

The average passenger weight was 240 pounds, which exceeded federal standards, the report said.
