A new report from the agency outlines recommendations stemming from a crash within a mile of Lexington Bluegrass Airport in Kentucky on Aug. 27. Comair Flight 5191 mistakenly took off from a shorter runway not designed for large aircraft departures. The airplane plummeted into a field shortly after takeoff, killing 49 of the 50 people on board.

The NTSB is recommending that the Federal Aviation Administration order airlines to create procedures for all crewmembers on the flight deck to get positive confirmation on the aircraft’s location at the assigned departure runway, and then to cross-check the information.

Understaffing at the control tower is also being looked into as a possible contributing factor to the crash of Flight 5191.
